Conducting comprehensive assessments under the Social Care (Self-directed Support) (Scotland) Act 2013 to identify eligibility for services. Investigating concerns and carrying out duties under the Adult Support and Protection (Scotland) Act 2007. Designing and implementing care packages, which may include home care, equipment/adaptations, or residential placement. Undertaking risk assessments for individuals with complex needs, challenging behaviours, or significant frailty. Managing the four options of SDS, allowing individuals to choose how their care budget is spent (e.g., collaborating with GPs, District Nurses, and Occupational Therapists to provide integrated health and social care).
