We are looking for a dedicated professional to join our team in Carlisle (with hybrid flexibility) to manage a specific caseload of assessments.   • Rate: £500 - £600 per day (DOE)

Key Responsibilities

  • You will be responsible for navigating the multidisciplinary assessment process to determine eligibility for NHS Continuing Healthcare.
  • Conduct comprehensive assessments for adults with primary, complex, intense, or unpredictable health needs.
  • Work collaboratively across multidisciplinary teams (MDT) to ensure care packages are fully funded and appropriate for home or residential settings.
  • Maintain accurate documentation to support funding decisions based on the National Framework for NHS CHC.
  • Support Children's services on an ad-hoc basis, providing a cross-continuum approach to care assessment.
