About the Role:
As a Social Worker in the Adults Mental Health Team, you will play a crucial role in assessing, planning, and coordinating support for adults with complex mental health needs. Working as part of a multi-disciplinary team (MDT), your responsibilities will typically include:
- Undertaking comprehensive assessments of individuals' care and support needs under relevant legislation (e.g., Social Services and Well-being (Wales) Act, Mental Health Act, Mental Capacity Act).
- Developing and implementing person-centred care plans, promoting recovery and independent living.
- Working in partnership with service users, their families, carers, and other professionals (e.g., psychiatrists, community mental health nurses, occupational therapists).
