About the Role:
We are seeking a dedicated Social Worker to facilitate timely and safe discharges from hospital, ensuring individuals receive the appropriate support and care in the community. In this fast-paced and critical role, you will be responsible for:
- Conducting rapid assessments of individuals in hospital to determine their social care needs upon discharge.
- Coordinating and planning safe and effective transfers of care from hospital to various community settings (e.g., home with support, residential care, nursing homes).
- Working collaboratively with hospital staff (doctors, nurses, therapists), service users, families, and community providers.
- Identifying and addressing potential barriers to discharge, advocating for the best interests of the individual.
- Ensuring compliance with discharge pathways and relevant legislation, including the Care Act 2014.
- Managing complex cases and making swift, well-informed decisions under pressure.
About You:
You will be a Qualified Social Worker with:
- Experience in hospital discharge planning, adult social care, or a fast-paced environment.
- A thorough understanding of relevant legislation, including the Care Act 2014 and its implications for hospital discharge.
- Excellent communication, negotiation, and problem-solving skills.
- The ability to work efficiently and effectively under pressure, prioritising multiple tasks.
- A strong commitment to person-centred care and ensuring positive outcomes for individuals.
- Current Social Work England registration.
