Social Worker - Adult Social Care - Hospital Discharge Team
Organisation: South Tyneside Council
South Tyneside are seeking a dedicated and passionate Social Worker to join their Adult Social Care team.
In this role, you will manage a varied case load, including adults with complex needs and those whose circumstances may place their personal liberty or safety at risk. You will work collaboratively with individuals, carers, and multi-agency partners to formulate outcome-focused, person-centred support plans that maximise abilities and safeguard well-being.
Key Responsibilities
- Undertake needs assessments and co-produce care and support plans tailored to identified outcomes and resource allocations.
- Promote strength-based approaches, direct payments, individual budgets, and community/universal resources to foster independence.
- Put the Making Safeguarding Personal ethos at the heart of your practice, undertaking Safeguarding Adults investigations in line with local procedures when required.
- Proactively manage your caseload, reviewing support packages regularly to ensure appropriate, cost-effective care is delivered for only as long as needed.
- Build strong networks within the local community and liaise effectively with health partners, agencies, and non-care services.
- Produce high-quality reports, maintain accurate records, and represent the service as a witness in court proceedings when required.
- Actively participate in reflective supervision/appraisals, stay current with legislation, and support less experienced team members.
To succeed in this role, you will need -
- A recognised Social Work qualification and active registration with Social Work England (or equivalent HCPC registration standards).
- A commitment to empowering individuals, positive risk management, and the Making Safeguarding Personal framework.
- Ability to manage complex caseloads, carry out adult safeguarding investigations, and navigate multi-agency environments.
- Excellent communication skills, strong report-writing abilities, and high standards of data quality.
- A collaborative mindset with a willingness to mentor colleagues and contribute to continuous service improvement.
