We are looking for a caring, motivated individual to join our friendly GP practice in a dual role combining Health Care Assistant (HCA) duties with Care Navigator and Administrative support. This is a varied and rewarding position supporting both the clinical team and our patients from taking bloods and conducting health checks to helping patients access the right care at the right time. You'll work across both clinical and reception/admin areas, making a real difference in patients' healthcare journeys.
Main duties of the job
- Clinical (HCA) Duties:
- Perform phlebotomy (blood taking)
- Record observations (e.g. blood pressure, pulse, temperature)
- Carry out ECGs and urinalysis
- Support chronic disease monitoring and NHS Health Checks
- Assist with wound care and dressings
- Support vaccination clinics (e.g. flu) if trained
- Maintain clinical areas and infection control standards
- Accurately document all care in patient records
- Greet and support patients, signposting to appropriate services
- Book appointments and manage patient recalls
- Handle phone, in-person, and online queries
- Liaise with external services and secondary care providers
- Process prescriptions and medication queries (as appropriate)
- Input and update patient data on clinical systems (SystmOne)
- Patient referrals to other services
- Assist with scanning, document workflow, and coding
- Support the delivery of QOF and other clinical targets
