We are seeking a compassionate and dedicated Complex Care Workers to provide specialised support to individuals with complex health needs in their own home on either a 1:1 or 2:1 basis. The successful candidate will deliver personalised care within clients' own homes, ensuring their comfort, safety, and wellbeing. This role offers an opportunity to make a meaningful difference in people's lives by delivering tailored care plans and fostering independence.
Responsibilities
- Deliver personalised care in accordance with individual care plans, ensuring all needs are met with dignity and respect.
- Assist clients with daily activities such as bathing, dressing, feeding, and mobility support.
- Support clients with medication management and health monitoring as required.
- Maintain accurate records of care provided and report any changes in client condition to relevant healthcare professionals.
- Facilitate effective communication with clients, their families, and multidisciplinary teams to ensure seamless care delivery.
- Drive clients to appointments or activities when necessary, ensuring safe transportation.
- Promote independence whilst providing necessary assistance in a caring and empathetic manner.
- Adhere to all health and safety regulations, including infection control procedures.
Experience
- Previous experience working within complex care is essential, such as mental health, brain injury, learning disabilities etc.
- Knowledge of care plans and their implementation is advantageous.
- Good IT skills for maintaining accurate documentation and communication records.
- Strong communication skills in English, both verbal and written, to effectively interact with clients, families, and colleagues.
- A valid UK driving licence and access to own vehicle is required.
