Rainbow Care Solutions is a long‑established, CQC‑registered home care provider with over 20 years’ experience. We’re recruiting Support Workers to support our Discharge to Assess (D2A) / Reablement service across Herefordshire.
Location: Herefordshire (community-based)
What you’ll be doing:
- Supporting people at home after discharge with reablement-focused care
- Assisting with personal care where needed, while encouraging independence
- Supporting daily living skills (washing, dressing, meal prep, mobility, routines)
- Following care plans and recording notes accurately
- Reporting changes in condition, concerns, or risks promptly
- Working respectfully with families and professionals as part of a wider team
What we’re looking for:
- Caring, reliable and professional approach
- Experience in home care is helpful, but not essential (full training provided)
- Good communication and respect for dignity, choice and independence
- Full UK driving licence and access to a vehicle (essential for Herefordshire)
- Right to work in the UK
