Rainbow Care Solutions, a long-established, CQC-registered home care provider with over 20 years' experience, is looking for an Assessment Coordinator to support our Redditch branch, covering Redditch and the surrounding areas.
About the role
As an Assessment Coordinator, you will play a key role in maintaining high standards of person-centred care and compliance. Your responsibilities will include:
- Carrying out regular spot checks on care staff to monitor quality and best practice
- Assessing new clients and completing initial assessments in line with company and CQC requirements
- Completing and reviewing care plans and risk assessments to ensure they are accurate, up to date and person-centred
- Conducting care reviews with clients and families, responding promptly to changing needs
- Carrying out supervisions and appraisals for care staff, providing support, guidance and feedback
- Working closely with the Care Coordinator and Branch Manager to address any concerns, incidents or complaints
- Supporting with on-call duties as required to ensure continuity and safety of care
