We've built a strong reputation for excellence, consistently achieving a 'Good' or higher rating from the CQC. We're currently seeking dedicated care assistants to join our team and help us continue providing outstanding in-home support.

We are recruiting Support Workers to support our Discharge to Assess (D2A) / Reablement service across Herefordshire. This role supports people returning home from hospital to recover safely and regain independence, doing with, not doing for.

What You'll Be Doing

  • Supporting people at home after discharge with reablement-focused care.
  • Assisting with personal care where needed, while encouraging independence.
  • Supporting daily living skills (washing, dressing, meal prep, mobility, routines).
  • Following care plans and recording notes accurately.
  • Reporting changes in condition, concerns, or risks promptly.
  • Working respectfully with families and professionals as part of a wider team.

What We're Looking For

  • Caring, reliable and professional approach.
  • Experience in home care is helpful, but not essential (full training provided).
  • Good communication and respect for dignity, choice and independence.
  • Full UK driving licence and access to a vehicle (essential for Herefordshire).
  • Right to work in the UK.
