About the role: Due to continued growth in our services, we are recruiting a Care Coordinator to help us deliver safe, person-centred care to our clients in Coventry and Warwickshire surrounding areas.
You will be responsible for: * Creating rotas and updating schedules to ensure safe staffing levels * Scheduling and coordinating care visits * Matching carers to clients based on skills, experience and needs * Liaising with clients, families, and external professionals * Supporting carers with day-to-day queries and problem-solving * Ensuring care plans and records are accurate and up to date * Participating in the on-call rota as required About you: Were looking for someone who: * Has proven experience in domiciliary (dom) care and/or care coordination * Is highly organised and able to work under pressure * Communicates clearly and compassionately * Is confident using digital systems for rostering and care planning * Holds a full UK driving licence and has access to a vehicle * Has the right to live and work in England * Shares our commitment to high-quality, person-centred care What we offer: 1.
