We are looking for a proactive and resourceful Care Coordinator Executive to manage incoming patient cases, perform basic triage, and coordinate doctor house calls. This role sits at the heart of our operations — requiring quick thinking, strong judgment, and the ability to manage multiple cases in real-time.
Key Responsibilities
- Clinical Triage & Case Intake
- Perform initial triage of incoming cases (calls, WhatsApp, referrals).
- Assess urgency and recommend appropriate care pathway.
- Ask structured questions to evaluate patient condition and risk.
- Operations, Scheduling & Dispatch
- Manage doctor schedules and coordinate house call routing.
- Prioritise cases based on urgency, location, and availability.
- Provide accurate ETA updates to patients and manage expectations.
- Handle last-minute changes and urgent requests.
- Client & Family Communication
- Act as the main point of contact for patients and families.
- Communicate clearly on care options, timelines, and service scope.
- Handle enquiries, feedback, and service issues professionally.
- Care Coordination & Case Management
- Coordinate cases from intake through to service completion.
- Ensure doctors receive accurate and complete case information.
- Monitor case progress and support continuity of care.
- Caregiver Coordination (Future Scope)
- Support matching of caregivers to client needs.
- Assist with scheduling and coordination of caregiving services.
- Liaise with caregivers on care plans and assignments.
- Documentation & Compliance
- Maintain accurate records of triage decisions, advice, and scheduling.
- Ensure proper documentation for operational and clinical tracking.
Requirements
- Diploma or Degree in Nursing (preferred) or Paramedic or relevant healthcare management field.
- Prior experience in clinic operations, home care, telemedicine, or care coordination is an advantage.
