Radis Community Care offers an opportunity for an Evening Field Care Supervisor (Driving essential) to join our team based in Chepstow.
You will need to be a driver with your own transport (car, motorcycle, moped, E‑Bike, etc.) — the work is not suited to non‑drivers.

About the role
As a Field Supervisor you will work a mix of mornings, afternoons and evenings to complete monitoring tasks such as reviews, spot checks, carer assessments, and the completion of risk assessments, MAR charts, and support plans.
You will be the face of Radis working in the community, ensuring people stay in the home they love, and you will also collaborate with the NHS, Social Services, GPs and other health professionals to meet all of our clients’ needs.
Responsibilities
- Conduct reviews, spot checks, and carer assessments.
- Complete risk assessments, MAR charts, and support plans.
- Collaborate with NHS, Social Services, GPs, and other health professionals.
- Ensure clients remain in their preferred homes while maintaining high standards of quality and professionalism.
Qualifications
- One year of experience in Health & Social Care.
- NVQ Level 2 in Health and Social Care (working towards Level 3).
- Good written and verbal communication skills and the ability to engage with staff, service users, and external agencies professionally.
Requirements
- Driving licence and access to a vehicle.
- Ready to undertake an enhanced DBS check.

- All supervisor hours are paid at an enhanced hourly rate.
- Care provided is paid at our care rate, with paid travel time between visits and paid mileage.
- On‑call work is compensated with an on‑call payment.
