Job Description

As a Hospice RN Case Manager you will be providing individualized,coordinated hospice care to bring safety and comfort to the patients and families. The holistic approach encompasses physical, emotional and spiritual well-being. You will have independence and flexibility in the field to visit patients one on one in their home environment. 

Hours:  Full-time |  Days | General working hours would be Monday - Friday from 8:00am - 5:00pm | Will also be a part of an On-Call rotation.

Territory:  The RN will have a general territory within our service area where they will provide in-home hospice care to the patients.  The territory for this position would primarily be in the Southern section of the Twin Cities Metro area.

A few must-haves for this position:

  • Licensed to practice as a RN in the state of MN.  License is active and in good standing.
  • Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate. 
    • If a new graduate, 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months or evidence of completion of a company-approved Preceptor Program.
  • One (1) year of hospice care experience preferred.
  • Able to travel within a geographic area serviced by the office. Reliable transportation, valid drivers license, and proof of auto insurance is required.
  • CPR Certification
