Our Physicians: All of our physicians are Board Certified and Fellowship Trained in Pain Medicine and Board Certified in Anesthesiology.

Responsibilities
- Identify and assess patient’s clinical issues and health care needs
- Record physical findings, and formulates plan and prognosis, based on patient’s condition and the doctor’s plan of treatment
- Evaluations and Re-evaluations of established patients
- Refers patients to Physician for consultation or to specialized health resources for treatment when needed.
- Discusses the case with Physician and other health professionals to prepare a comprehensive patient care plan
- Performs pain management injections for specific diagnoses according to practice protocols
- Prescribe/Order as medically indicated:- Diagnostics- Therapeutics- Medication- Pain Management Procedures- DME
- Eagerness and ability to work independently as well as part of a team with flexibility and willingness to learn and take initiative on variety of tasks and projects
- Supports the vision and culture of the organization. Demonstrates personal commitment through active involvement in the performance improvement process
- All other duties as assigned
Required SkillsSkills and Abilities:
- Highly proficient with computers especially: EMR Software, Meaningful Use, Microsoft Word, Excel, Outlook
- Ability to manage high levels of stress and capable of making sound difficult decisions
- Very structured, organized, consistent, pleasant, cheerful, yet professional personality, who will earn the respect of patients and co-workers immediately
- Must be incredibly punctual. Great attendance, required
Qualifications, Education, and Experience (APRN):
- Active, valid (in state of clinic) (APRN) Nurse Practitioner certificate, required
- DEA Registration, preferred
Qualifications, Education, and Experience (PA-C):
- Must have a valid, unrestricted professional license in state of Indiana and be able to obtain clinical privileges to practice medicine
Physical Requirements:
The physical demands of this position will include sitting and standing with occasional light to medium lifting. Ability to help lift patients and transport in wheelchair from clinic to parking lot. An employee must meet these requirements to successfully perform the essential duties of this job. Reasonable accommodation will be made in accordance with ADA rules and regulations.
