Admissions (Start of Care) Visit RN – Home Care:  Enjoy independence and flexibility in the field to care for patients in their home environment.  The Admissions Visit RN will provide head to toe assessments, care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.   

Hours:  Full-time | Days | Monday – Friday |  Holiday rotation (work approximately two holidays per year)  |  Weekend rotation may apply dependent upon staffing levels.

A few must-haves for this position:

  • Licensed to practice as an RN in the state of Minnesota. License is active and in good standing.
  • Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance required.
  • Minimum of twelve (12) months experience working in an acute care or home health environment.
  • Evidence of current CPR certification.
  • Experience in Home Care to include OASIS; minimum one (1) year experience preferred.

Some functions of what you would do:

  • Completes admissions and/or visits for the Home Care team.
  • Responsible for delivering nursing care to patients as directed by the RN Case Manager and the physician’s plan of care.
  • Ability to assess and determine appropriate wound care, including obtaining MD orders.
  • Provides accurate and timely documentation of patient services to reflect and create the plan of care.
  • Provide and determine treatments for the patients.
