Are you an experienced Social Worker looking to make a real difference to young people transitioning into adulthood? Our client in Bristol city is looking for a skilled Social Worker to join its dedicated Transitions Team, supporting young people aged 14-25 as they move from children's to adult services. Working within a multidisciplinary team of Social Workers, Occupational Therapists and Social Care Practitioners, you'll play a key role in helping young people achieve greater independence and positive long-term outcomes.
What will your responsibilities be?
- Complete Care Act assessments, support plans and reviews
- Support young people and families through the transition to adult services
- Promote independence through strengths-based practice and early intervention
- Commission services and identify community-based support, assistive technology and equipment solutions
- Work collaboratively with health, education and social care professionals
Qualifications & Experience:
- Social Work England registration
- Experience completing Care Act assessments and support planning
- Strong understanding of transitions and strengths-based practice
- Ability to manage a complex caseload in a fast-paced environment
Working Arrangements:
- Minimum 2 days per week in the office
- Wednesday is the designated team day
- Travel outside Bristol may be required to visit young people placed out of county
