Liquid Personnel is seeking a dedicated qualified social worker to join its client's busy older persons team in Nottinghamshire.

Responsibilities:

  • Manage a complex caseload, conducting assessments, support planning, and reviews to meet individual needs and promote independence.
  • Identify opportunities for Reablement services and use a Strength‑Based Approach to support well‑being and delay long‑term care.
  • Contribute to Continuing Healthcare (CHC) assessments, coordinate service provision, and ensure effective outcomes.
  • Identify community resources and maximise individual assets to support preventative strategies.
  • Liaise with providers, offer professional advice to individuals and carers, and undertake safeguarding responsibilities as a Safeguarding Officer.

Qualifications & Experience:

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving license
  • Post‑qualified experience
  • Degree level or equivalent in Social Work
