Responsibilities

  • Support individuals following hospital discharge by conducting timely reviews.
  • Participate in Decision Support Tool (DST) meetings to assess ongoing care needs.
  • Involved in Adult Safeguarding work.
  • Contribute to the team’s duty rota, handling urgent cases and initial assessments after gaining experience.

Qualifications & Experience

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
