Responsibilities
- Support individuals following hospital discharge by conducting timely reviews.
- Participate in Decision Support Tool (DST) meetings to assess ongoing care needs.
- Involved in Adult Safeguarding work.
- Contribute to the team’s duty rota, handling urgent cases and initial assessments after gaining experience.
