Cheshire East (Macclesfield) Council is seeking a dedicated Social Worker to join their dynamic Hospital Discharge Team. This role offers the opportunity to make a meaningful impact on patients' lives by ensuring smooth transitions from hospital to home.
Responsibilities
- Assess the needs of individuals being discharged from hospital to develop tailored care plans that promote independence and well-being.
- Collaborate with hospital staff, families, and external providers to coordinate effective discharge planning and support packages.
- Conduct risk assessments and adhere to safeguarding protocols to protect vulnerable adults.
- Implement the Care Act, Mental Capacity Act, and other relevant legislation to ensure compliance with statutory responsibilities.
- Facilitate access to community resources and services, supporting individuals to reintegrate into their home or alternative care settings.
- Maintain clear, accurate, and timely records, adhering to council policies and best practice guidelines.
- Engage in reflective supervision and contribute to service improvement initiatives.
