Responsibilities
- Work with Health colleagues to reduce hospital attendance.
- Complete visits (including same-day response).
- Conduct Care Act and short-term assessments to promote independence.
- Provide Social Work expertise in a multi-disciplinary team.
- Maintain an office-based role requiring a clean driving license and transport.

Qualifications and Experience
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
