An exciting job role has recently become available for an Adult Social Worker. They are looking for a new member to join their Adult's Integrated Neighbourhood Team based in Bury.
Responsibilities:
- Undertaking assessments and reviews under the Care Act 2014
- Holding a caseload and supporting the duty function
- Evaluating the needs of individuals, ensuring they receive appropriate care and support
- Conducting thorough assessments and developing care plans
- Regularly reviewing the progress and effectiveness of these plans
- Collaborating with other professionals and agencies to provide comprehensive support
- Promoting the well-being and independence of individuals
Qualifications and Experience:
- Social Work England registration
- Eligibility to work in the UK
- Qualification in Social Work at degree level
- Post-qualified experience
- Experience of assessments, support planning, duty, CHC, MCA and best interest, working with adults of working age
