An exciting job role has recently become available for a dedicated and experienced Adult Social Worker to join our client’s Discharge 2 Assess team based in Oxfordshire. This is an excellent opportunity to work within a dynamic and supportive team environment, contributing positively to the lives of service users.

Responsibilities:
- Conduct comprehensive assessments of service users to determine their needs and eligibility for services.
- Develop, implement, and review care plans in collaboration with service users and their families.
