We are looking for a passionate and skilled Adult Social Worker to join the Preparation for Adulthood team. This role focuses on supporting young people with disabilities or complex needs as they prepare for the transition to adult life. The successful candidate will work closely with individuals, families, and multi-disciplinary teams to facilitate a smooth and successful transition.
What will your responsibilities be?
- Conduct thorough assessments of young people's needs and develop individualized preparation plans.
- Provide guidance, information, and support to young people and their families about available services and resources.
- Collaborate with healthcare providers, educational institutions, and other agencies to coordinate comprehensive support.
- Regularly review and update preparation plans to ensure they remain effective and relevant.
- Maintain detailed and accurate case records and documentation.
- Participate in team meetings, training sessions, and professional development opportunities.
Qualifications and Experience
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
