Queen Square Medical Practice is seeking an experienced and motivated Practice Nurse with a particular interest and expertise in diabetes management to join our friendly and progressive clinical team., The successful candidate will participate in leading and supporting the delivery of diabetes care within the practice, ensuring patients receive evidence based assessment, treatment, education and ongoing management.,
- Diabetes Management
- Participate in leading diabetes clinics within the practice.
- Undertake comprehensive diabetes reviews, including annual assessments.
- Support achievement of QoF and other quality indicators relating to diabetes care.
- Interpret and act upon investigation results.
- Optimise diabetes management in line with current NICE guidance.
- Provide insulin initiation and titration support where appropriately qualified.
- Deliver patient education to promote self-management and improve health outcomes.
- Identify and proactively manage patients at risk of complications.
- Long-Term Condition Management
- Manage patients with chronic conditions including:
- Diabetes
- Hypertension
- Cardiovascular disease
- COPD
- Asthma
- CKD
Undertake clinical assessments and formulate management plans. Monitor treatment effectiveness and escape concerns where appropriate.
This is an exciting opportunity for a skilled nurse looking to participate within a leading role in the delivery and development of high quality diabetes and long term condition services within a well established and patient focused practice. The successful candidate will have substantial experience in general practice nursing and be confident managing patients with diabetes and other chronic diseases. Independent prescribing would be advantageous but is not essential.
Queen Square Medical Practice is a busy independent GP Practice based in Lancaster with a patient list of15500. We have 9 GP Partners and 5 Associate GP's who are supported by ANPs, paramedics, Pharmacists, Nurses, HCA's and Admin team. We also have a branch site in Caton.
