The successful candidate will be based within the Rapid Assessment and Frailty Team and the role will involve delivering high quality assessment, treatment and advice for patients within our emergency areas. You will concentrate on admission avoidance and facilitating early discharge.

You will manage patients with diverse and often complex needs whilst working as part of a supportive team. This role will include weekend working as part of an agreed rota.
