A fantastic opportunity has arisen for an enthusiastic pro-active person to work as a Discharge Planning Assistant within the Complex Discharge Team.
Responsibilities
- Support the wards to discharge their patients in a timely manner.
- Improve outcomes for patients being discharged once medically fit.
- Early identification of potential needs and additional support to achieve a short length of stay and reduce the risk of potential readmission.
- Demonstrate trust values and behaviours.
- Have relevant previous hospital experience and a desire to progress in health care.
- Be willing to work towards completing training that enhances knowledge and skills.
- Have a flexible approach to meet the needs of the service.
- Work with the Complex Discharge Sisters and Matron to promote a discharge ethos amongst the wider MDT.
- Focus directly on the needs of the patients requiring support and facilitation to be discharged from the acute hospital.
Additional Responsibilities
- Develop close working relationships with ward teams and provide administrative support to the multi-professional team.
- Act as an expert resource for patients, their carers, and the multi-professional team regarding the discharge process.
- Drive a team culture alongside colleagues working together.
- Act as an advisor regarding discharge to community settings.
