Are you passionate about making a real difference in people's lives? We are looking for a compassionate and proactive Care Coordinator to join East Warrington Primary Care Network (PCN), working collaboratively across our three GP practices to support patients in accessing the care and services they need.
In this rewarding patient-facing role, you will play a key part in improving health outcomes and reducing health inequalities within our local community. Working with both adults and children, you will help individuals navigate a range of healthcare pathways, initially focusing on neurodevelopmental and cancer care pathways, while also supporting other priority areas in response to patient and practice needs. This is an opportunity to have a meaningful impact on the lives of some of the most vulnerable members of our community, including individuals waiting for neurodevelopmental assessments, people living with long-term conditions, and those navigating cancer care pathways. No two days are the same, offering a varied and fulfilling workload where your support can genuinely change lives.
Main duties of the job
- Supporting patients and their families through complex healthcare journeys, providing practical guidance, emotional support, and reassurance.
- Helping individuals better understand and manage their own health and wellbeing.
- Connecting patients with appropriate health, social care, community, and voluntary sector services.
- Working closely with GPs, healthcare professionals, and non-clinical staff to coordinate care and ensure patients receive timely support.
- Contributing to proactive healthcare initiatives, including improving screening uptake, NHS Health Checks, blood pressure monitoring, and support for people living with long-term conditions.
- Providing administrative and coordination support to help deliver high-quality, patient-centred care.
About us
East Warrington PCN is one of five Primary Care Networks serving Warrington. Together, our three member practices – Birchwood Medical Centre, Fearnhead Cross Medical Centre and Padgate Medical Centre – provide high-quality primary care services to over 31,500 patients. Since the introduction of the Network Contract Directed Enhanced Service (DES) and the NHS Additional Roles Reimbursement Scheme (ARRS), we have continued to expand our multidisciplinary workforce to enhance patient care and improve access to a wide range of healthcare services. Our dedicated team includes: Paramedics, Social Prescribing Link Workers, First Contact Physiotherapists. Our PCN is led by experienced and committed Clinical Directors, supported by a Strategic Manager, Practice Managers, GP Partners, and a collaborative multidisciplinary team.
In addition to being part of East Warrington PCN, our practices are members of Quay Healthcare, a not‑for‑profit organisation owned by Warrington GP practices. Quay provides operational, business, and administrative support to Primary Care Networks, as well as delivering a range of additional healthcare services across the locality. Employment for this role will be through Quay Primary Healthcare. While your day‑to‑day role will involve working closely with colleagues across the PCN and our member practices, Quay Healthcare will act as your lead employer.
Job responsibilities
- Maintain and track a caseload of patients who require support across a variety of medical care pathways.
- Have a positive, empathetic and responsive conversation with the person, their family and carer(s) about their needs.
- For neurodevelopmental pathways, provide support to the individual and practice to ensure documentation and coordination of referral pathways to ensure the patient is accepted for assessment.
- Support improvement in cancer screening uptake, liaising with external agencies as appropriate.
- Support improvement in recording of blood pressure readings, and supporting improved uptake of NHS Health Checks.
- Work closely with Practice Managers, GPs, social prescribers, and other roles in accordance with the medical pathway.
Patient & Family Support
- Act as a named point of contact for practices.
- Communicate with empathy when dealing with patients and their carers.
- Signpost to local and national support services (e.g. charities, peer groups) and develop a directory of contacts.
Assessment Preparation
- Support practices by completion of forms at practice level for any referrals.
- Support patients and families to complete appropriate referral questionnaires and forms.
- Liaise with Carers, or other professionals where appropriate.
- Act as a bridge to other services across Warrington in accordance with patient needs.
- Build relationships with other services to maintain positive relations on behalf of the PCN.
- Act as a bridge between primary care, specialist services, or under the right to choose pathways.
- Communicate with empathy for non NHS referrals.
- Provide updates on referral progress where available.
- Support patient understanding of the pathway.
Record Keeping & Administration
- Accurately document patient contacts and interventions in GP clinical systems.
- Maintain up‑to‑date records of referrals and outcomes.
- Support data collection for service evaluation and improvement.
- Develop a directory of services for the role, maintaining up‑to‑date information on alternative services in the community and town.
Personal and Professional Development
- Keep abreast of national and local guidance on PCNs.
- Take responsibility for own development, learning and performance.
- Participate in an annual individual performance review.

- Support PCN admin and project work.
- Developing and maintaining effective working relationships both within and outside the practice, PCN and locality.

- Assist the PCN to achieve successful multidisciplinary teams (MDTs) across East Neighbourhood (when appropriate).
Other Duties and Responsibilities
- Help people to manage their needs through answering queries, making/managing appointments, and ensuring that people have good quality written or verbal information available to them to help them make choices about their care.
- Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, and other primary care professionals; and support the coordination and delivery of MDTs.

- Demonstrate understanding of the implications of national priorities for the PCN.
- Bring to the attention of the Practice Manager any clinical incidents, and participate as required in any investigation and learning exercise.

Education, Training and Development
- Participate in relevant training programmes that are consistent with the role.
- Maintain statutory and mandatory training record.

Location
The successful candidate will be located within GP Practices in East PCN, and travel to and from the GP Practice surgeries and Orford Jubilee hub.
