Waythrough is seeking an Assessment and Engagement Worker to join the Reconnected to Health team at HMP Holme House.
You will assess, plan recovery, and help individuals access support to improve health outcomes within a prison setting.
You will work with healthcare professionals, prison staff and partner organisations to provide person-centred support, manage a caseload, and deliver group and one-to-one interventions as part of a multidisciplinary team.
