East Bedford PCN is a primary care network of three Practices delivering services for our combined population of 46,000 registered patients. We now have an opportunity for an experienced manager to join our team. Working hours will be 16 - 22 hours per week (flexible). The successful candidate will have the ability to contribute to positive changes in local healthcare, and to innovate to support general practice at a time of rapid change.
Place of Work: Remote working but will be required to attend ad hoc meetings on site at London Road Surgery and Putnoe Medical Centre.
Main duties of the job:
- The PCN Manager and Transformation Lead will work with clinical and support teams to identify opportunities for, and lead, service improvements.
- The post holder will work closely with local partner agencies to ensure plans and activity are aligned to the ICB priorities, including:
- Developing Neighbourhood Teams
- Optimisation of digital ways of working
- Development of multi-professional teams
- Quality Improvement
As the PCN Manager and Transformation Lead, you will primarily work remotely. However, you will be required to attend on-site meetings on an ad-hoc basis, depending on operational needs. You will also be expected to be available for weekly 1:1 meetings with the Clinical Director. The schedule for these meetings will be arranged collaboratively following your appointment.

East Bedford PCN is a primary care network of three Practices delivering services for our combined population of 46,000 registered patients. Our aim is to bring together local health and care organisations to deliver more joined up, patient-centred services. We manage a range of services to the member practices, including physiotherapy, home visiting service, social prescribing, care co-ordination and mental wellbeing services. Other services may be added in the future.
As part of the NHS Long Term Plan, East Bedford PCN is working collaboratively with providers and patients to co-produce solutions to the challenges we face and maximise the impact of our services on quality and outcomes of patient care.
Job responsibilities:
- The PCN Manager and Transformation Lead will work with clinical and support teams to identify opportunities for, and lead, service improvements.
- The postholder will work closely with the ICB to ensure local plans and activity are aligned to the ICB Primary Care priorities, including:
- Developing Neighbourhood Teams
- Optimisation of digital ways of working
- Recruitment, retention & development of multi-professional teams
- Quality Improvement
Business and service development:
- A key objective of the PCN Digital & Transformation Lead role is to support practices to deliver ongoing improvement to services. The role will require experience in business/service improvement and a thorough understanding of general practice systems and processes.
- This expertise will be used to:
- Work with the member Practices to drive improvements in the delivery of care and improve patient and staff experience in the practices within the PCN.
- Develop and manage a plan for the PCN including managing risks, considering the potential impacts on the wider organisation, and determining resource requirements.
- Use data to understand demand, capacity and activity and drive improvements in patient experience of access, operational efficiency including better matching capacity to need and staff experience.
Finance:
- Ensure that PCNs resources are managed for maximum effectiveness through:
- Planning and monitoring income and cash flow to ensure that income is maximised and that funds are available as required.
- Ensuring all claims are submitted in good time and payment made and received.
- Ensuring the allocation of resources between the Practices is equitable.
Information and data management:
- Use data to drive improvement and assess impacts:
- Use evidence to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made.
- Bring together data from a variety of sources and use information to develop insights into working practices.
- Interpret data and highlight variation to support open discussion about where variation is and is not warranted, and help Practices to understand the limitations of particular data sets and where other information may be available to supplement understanding of issues.
Staff and Operational Management:
- Act as first point of contact for Practices/Practice Managers with regard to delivery of PCN projects/services.
- Manage contractual requirements on behalf of the PCN.
- Co-ordinate the delivery of enhanced services and other shared service.
- Management lead for PCN centralised team, including:
- Recruitment, induction, training and appraisal.
- Ensuring the team remain happy and well-motivated through effective leadership and communication.
- Deal with any performance issues.
- Recording and management of HR data including pay, holiday and sickness of PCN staff.
- Management of the clinical rota for PCN staff.
- Develop protocols and policies to provide clarity and support safe practice.
- Support staff to access relevant training and/or professional development opportunities that will support achievement of agreed improvements.
- Support the development and activities of the PCN Training Team.
- Monitor significant events and complaints.
- Organise PCN events as required.
Digital & Transformation:
- Review and improve the PCNs digital maturity.
- Improve adoption of new technology to enhance patient access and experience and increase PCN productivity.
- Support population health management and improvements in care quality.
- Support the effective adoption of national and local initiatives, including integrated working at neighbourhood and place level to improve access to services for patients.
- Ensure that digital and operational PCN development is aligned with ICB and national strategies.
Communication and engagement:
- Build good working relationships with internal and external stakeholders.
- Identify best practice from other areas that could be applicable to the local PCN.
- Work in partnership with ICB Primary Care & Digital Teams and other service delivery organisations to plan and deliver changes that will improve working between general practices and other primary care and non-primary care services.
- Lead or participate in relevant working groups and other fora across the PCN to agree changes, share learning, discuss challenges, and resolve issues.
Policy and service development:
- Maintain a good knowledge of emerging policies from the Department of Health and other relevant government departments, and support PCN Board and staff to interpret these locally.
- Work with the ICB to support the development of primary care development strategies.
The post holder will be expected to undertake other duties as may reasonably be required by the Clinical Director or Deputy Clinical Director, in liaison with the Practice Managers of Putnoe Medical Centre and London Road Surgery.
Person Specification:
- Qualifications: Degree or equivalent level education OR ability to demonstrate competency at this level.
- Experience of driving change including translating strategic objectives into tangible plans for delivery.
- Project management background and qualification desirable.
- Ability to use data and insight to both drive change and to understand and adapt programmes of work.
- Excellent organisational skills, including financial management.
- Ability to work and make decisions autonomously, and to identify and act on challenges to move delivery forward, potentially working to tight timescales.
- Excellent communication skills and an ability to engage successfully with a wide range of people at all levels within the organisation.
- A clear and ongoing commitment to improving NHS services for the benefit of patients.
- Self-motivated and flexible.
- Experience of working in general practice.
- Demonstrates a clear understanding of how general practice works and the distinctive roles of Practitioners in the sector.
- Experience of championing diversity.
