The successful Clinical Pharmacist will work as part of a collaborative primary care team, playing a key role in improving patient outcomes and supporting safe, effective prescribing. Key responsibilities include:
- Conducting structured medication reviews and medicines reconciliation
- Supporting patients with long-term conditions and complex medication needs
- Managing repeat prescriptions and prescribing queries
- Providing clinical advice to GPs, nurses, and other healthcare professionals
- Supporting QOF, audits, and prescribing optimisation initiatives
- Delivering patient-facing clinics (face-to-face and telephone)
