The role

You will work as part of the wider Primary Care team, assessing and managing patients with a broad range of acute, minor illness and long-term conditions.

A key part of the position will be undertaking home visits, providing face-to-face clinical assessment and treatment to patients who are unable to attend the surgery.

You will have the support of the wider GP and multidisciplinary team while still having the autonomy to utilise your advanced clinical skills.

The position can be tailored around the successful candidate, with both full-time and part-time hours available.

Key responsibilities

• Undertake autonomous clinical assessments

• Assess, diagnose and manage patients presenting with acute and minor illnesses

• Undertake planned and urgent home visits

• Manage patients with long-term conditions where appropriate

• Prescribe within your scope of practice where qualified

• Make appropriate referrals and safety-net patients

• Provide clinical advice and education to patients

• Work closely with GPs, Practice Nurses and the wider multidisciplinary team

• Maintain accurate and thorough clinical records

• Work within professional and clinical competencies

• Contribute to the continued development of the practice

About you

Ideally, you will have previous experience within Primary Care, General Practice, Urgent Care or a similar autonomous clinical environment.

You should be confident assessing undifferentiated patients and making appropriate clinical decisions.

Primary Care experience is desirable, alongside appropriate prescribing and minor illness qualifications where applicable.
