Advanced Clinical Practitioner (ACP) to join its friendly and supportive multidisciplinary team.

The Opportunity
The successful Advanced Clinical Practitioner will work autonomously, managing a varied clinical caseload whilst working closely alongside GPs, Practice Nurses, Clinical Pharmacists, Paramedics and the wider Primary Care team.
This role offers excellent clinical autonomy, protected time to deliver high-quality patient care and the opportunity to further develop specialist interests within a supportive Training Practice environment.
Key Responsibilities
- Independently assess, diagnose and treat patients presenting with acute and minor illness.
- Manage same‑day and routine appointments.
- Request, interpret and act upon diagnostic investigations.
- Prescribe safely and effectively (V300 Independent Prescriber desirable).
- Develop comprehensive management plans and provide appropriate follow‑up.
- Refer patients to secondary care and community services where appropriate.
- Work collaboratively with the wider multidisciplinary team to ensure outstanding continuity of care.
- Promote health education, disease prevention and long‑term condition management.
- Maintain accurate and timely clinical records using the practice clinical system.
- Participate in clinical meetings, audits and quality improvement initiatives.
- Candidate Requirements
- Registered Healthcare Professional (NMC or HCPC).
- Qualified Advanced Clinical Practitioner (Master's level or equivalent).
- Experience working within Primary Care or General Practice is highly desirable.
- Confident in autonomous assessment, diagnosis and clinical decision‑making.
- Independent Prescriber qualification (V300) is highly desirable.
- Excellent communication and interpersonal skills.
- Passionate about delivering exceptional patient‑centred care.
- Committed to continual learning and professional development.
