28 hours per week, 4 days per week (Monday to Thursday OR Tuesday to Friday)

Role Purpose:

The Community Hub is part of the Early Intervention & Prevention services in Stockport. The Stockport Support Hub.

Pure’s Wellbeing & Engagement Workers will provide 1:1 support to individuals aged 18 and over in the Stockport area.

This role involves assisting clients with issues related to social isolation, health and wellbeing. Introduction support to leisure activities, volunteering opportunities, training & learning, and community involvement, while helping them address any underlying challenges they may face.

Our goal is to help people connect with their communities and engage in opportunities that lead to a fulfilling and meaningful life.

Additionally, the post holder will be involved in facilitating the Community Hub’s weekly social groups, projects, and collaboration with other organisations & members of the Network and the community to achieve the service\’s objectives.

Responsibilities

  • To plan and prioritise personal targets for each of the clients referred to the service
  • To be an integral part of Stockport Support Hub. Early Help & Preventive Services.
  • Support and match clients to suitable activities and opportunities as identified.
  • Creative problem solving.
  • Offer individual support to clients over relevant contractual timeframes.
  • Update personal support plans for each client.
  • Liaise with partner organisations and the wider Voluntary, Community and Health Sector organisations within Stockport that may be able to provide additional support to clients.
  • Enhance people’s lives through enabling participation.
  • Maximise client independence.
  • Identify natural support networks either in the community or in people\’s own networks of support.
  • Work in partnership with all project partners as part of the Community Hub and Pure.
  • To meet agreed targets and report on these at agreed periods.
  • To keep and maintain records of activity, both written and electronic, and produce regular monitoring information as required.
  • To participate as appropriate in the monitoring and evaluation of the Community Hub.
  • To work as an active member of the Stockport Third Sector, attend meetings, and contribute to the ongoing development of the service.
  • To undertake and contribute to appropriate training.
  • To participate in a formal programme of 1:1 meetings and annual appraisals.
  • To participate in conferences, courses, and training events as required.
  • To maintain an up-to-date knowledge of relevant legislation and service developments and to adhere to all appropriate policies, procedures, and guidelines.
  • To uphold, understand, and adhere to all policies and procedures identified by Pure Innovations Ltd.

  • To show a willingness to raise any concerns with management.

Requirements

Relevant experience within the third sector, with a track record of supporting individuals to reach positive goals.

Ability to work independently & oversee personal time management.

Essential to have access to a vehicle for use during work hours and be willing to obtain business insurance & transport clients.

Time Scales

  • Closing date for applications will be 12th September 2025.
  • In-person interviews will be held on the 22nd and 23rd of September 2025 at our Head Office in Hazel Grove.
