As a PCN Digital Care Coordinator you will be a key member of the Care Coordinator team, focusing on supporting PCN multi-disciplinary teams to coordinate and deliver effective care for a variety of cohorts of patients. The successful candidate will need to be organised, self‑motivated, flexible, and comfortable with adapting their workload to respond to changing and conflicting priorities. You will be expected to support the 6 member practices at any given time and therefore building effective relationships is key to this role as well as being adaptable to change. The role will be based at Wareham Health Centre working 15 hours per week, ideally over 3 days.

Main duties of the job

  • Although you will be aligned to supporting digital services, you will be required at times to support other services within the PCN such as Early Cancer Care, Diabetes, Pharmacy, FCP, Spirometry and INTs.
  • You may be required to travel to our member practices to support the rollout of digital projects; therefore, the post holder would need to be a car driver and have access to a car; mileage for this will be reimbursed at the agreed rates.
  • You will also be provided with a laptop and mobile phone to fulfil the role requirements.
  • Training is a key aspect of this role; the individual must be enrolled in, undertaking, or qualified from appropriate training set out by the Personalised Care Institute.
  • There is also the prospect to enlist in opportunistic training to improve and develop your level of knowledge to support your job outcomes.
  • Full training to undertake this role will be given during an induction.

Purbeck PCN is made up of 6 member practices and works closely with the wider locality to improve and enhance patient care. Purbeck PCN is committed to delivering an integrated neighbourhood team approach within the community and works to improve patient access to local services, by working with many sectors including the voluntary sector, mental health services, Help & Care, and Dorset Mind.

Job responsibilities

  • Work closely with PCN colleagues to identify patients who currently do not use digital channels for healthcare access, promoting digital inclusion and encouraging the wider use of the NHS App.
  • Use available data sources to help the multidisciplinary teams understand the health and well‑being profiles of our residents and to identify specific groups of patients and arrange care for them.
  • This will include undertaking searches to identify individuals who meet particular criteria, for example patients requiring blood pressure monitoring.
  • Assist the PCN to deliver improvements to the services we provide our patients with relevant data and data analysis.
  • Support the PCN with coding activity appropriately and develop reports to demonstrate PCN activity.
  • Assist the PCN member practices to engage with hard‑to‑reach populations and to reduce health inequalities.
  • Take a lead in the deployment of new technologies and education of patients.
  • Run regular proactive reports to identify patients who may benefit from new services/new ways of delivering services and ensure appropriate referrals are made.
  • Work with colleagues and partner organisations to promote digital inclusion, for example, by demonstrating to and educating colleagues about how to optimally use digital technologies and data/intelligence.
  • Develop social media content for the PCN including website, Facebook – promoting services and health campaigns.
  • Provide data administration support to the PCN Leadership team and member practices as required.
  • Support any reasonable requests from the PCN Leadership team.
  • Act as an ambassador for use of digital healthcare technology with both patients and colleagues.
  • Support PCN and member practices with handling and analysing data and developing our digital maturity.
  • Support partner practices to maintain and monitor data coding and quality on clinical systems, ensuring coding is compliant and consistent.
  • Run system reports and extract data as required by the PCN for activity reporting, using SystmOne clinical system, PCN digital dashboard and other data sources.
  • Be a point of contact for digital enquiries and be an active member of the NHS Dorset Digital CoP.
  • Under the direction of the PCN Leadership team, support the implementation of digital projects across the PCN and member practices.
  • Responsible for PCN service rota management within the SystmOne clinical system, including setting up new rota templates, taking the appropriate action for holiday/rota change requests, housekeeping of the clinical system including task monitoring, auto‑apply rota management and general troubleshooting.
  • Responsible for the management and oversight of the PCN digital inbox, actioning tasks accordingly and signposting where appropriate.
  • Ensuring internal processes and procedures are adhered to by all staff members.

Person Specification

Experience

  • Essential: Ability to develop and maintain good working relationships and communicate with a variety of audiences in a professional manner.
  • Experience working across several locations, autonomously and as part of a wider team.
  • Good working knowledge of and experience using SystmOne, including running reports, analysing, and manipulating data and working within appointment systems.
  • Ability to demonstrate effective organisational skills and knowledge of working to time‑plans.
  • Intermediate knowledge of Microsoft Office packages including Word, Outlook, PowerPoint, and Excel.
  • Previous experience working in similar role with strict guidelines, organising meetings and other aspects of project work.
  • Experience of supporting service improvement using data/digital platforms and negotiating with relative teams in the process.
  • Flexibility in approach to tasks undertaken.
  • Excellent written, verbal communication and interpersonal skills.
  • Ability to use initiative, work independently and as part of a team.
  • Must be a car driver and have access to a car.

Desirable

  • Safeguarding Level 3 in Adults & Children & Young People.
  • Undertaking training or qualified from a care coordination or health coach course by a recognised Personalised Care Institute provider.
  • Experience working in general practice/healthcare organisation.

Qualifications

Essential

  • Educated to at least GCSE level or equivalent.
  • SystmOne experience.
  • Good understanding of health and social care.
  • Digitally literate with advanced working knowledge of digital system and processes.
  • Proven experience of working with the general public in a similar role.
  • Experience of working in a service provision and/or healthcare organisation.

Desirable

  • Health and wellbeing qualification or equivalent experience.
  • Experience working with vulnerable adults.
  • Microsoft applications including MS Teams.
  • Project management experience/setting up of a new service.
  • Previous experience in a process role.
