Job Description:
We are seeking a Band 6 Locum Physiotherapist to join our Community Intermediate Care and Reablement Service. In this role, you will provide essential physiotherapy input to patients within the Reablement and Adult Social Care caseload. Your expertise will be vital in supporting patients’ recovery and enhancing their functional independence in the community.
What your responsibilities will be for this Physiotherapy role?
As a Band 6 Locum Community Physiotherapist You will conduct comprehensive assessments of patients under the Reablement and Adult Social Care programs to identify their physiotherapy needs. You will develop and implement individualized treatment plans that promote recovery and improve mobility. You will manage simple occupational therapy cases, demonstrating a holistic approach to patient care. You will provide cross-prescribing as necessary, ensuring seamless care and support for patients. You will monitor patient progress, adjusting treatment plans as needed to achieve optimal outcomes. You will collaborate with multidisciplinary teams to ensure coordinated care and effective communication among all stakeholders. You will maintain accurate and detailed documentation of patient interactions and treatment plans in compliance with professional standards and organizational policies.
Qualifications and Experiences:
- Must be a registered Physiotherapist with the Health and Care Professions Council (HCPC).
- Must hold a relevant degree in Physiotherapy.
- Must have experience in community physiotherapy settings, particularly in intermediate care and reablement services.
- Must be able to cross-prescribe and manage simple occupational therapy cases.
- Must possess strong communication and interpersonal skills.
- Must be able to work independently and collaboratively within a multidisciplinary team.
- Must demonstrate a commitment to providing patient-centered care and improving patient outcomes.
