We are seeking a highly skilled and motivated Band 6 Physiotherapist to join our esteemed client in Hounslow. You will work collaboratively within a multidisciplinary team and your caseload will consist of supporting triage referrals for both the Discharge Hub and Home First services.
Key Responsibilities
- Triage referrals via telephone, assessing patient needs, and coordinating appropriate care pathways.
- Facilitate timely discharges and support patients transitioning from hospital to home.
- Ensure safe and effective care planning using clinical judgement and communication skills.
Qualifications and Experience
- HCPC registration.
- Bachelor’s degree in Physiotherapy or a related field.
- Experience in community or discharge planning settings is desirable.
- Strong clinical reasoning and decision-making skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a team.
