We are seeking a highly skilled and experienced Advanced Clinical Pharmacist to join our team on a temporary basis. This role involves providing expert pharmaceutical advice to clinical teams, with a focus on cancer services. The position is available for a 3-month booking, and both full-time and part-time applicants will be considered.
Duties and Responsibilities:
- Provide expert pharmaceutical advice to clinical teams, ensuring that medication regimens are aligned with the latest clinical guidelines and evidence-based practices.
- Ensure the safe and effective use of medications by monitoring for potential drug interactions, side effects, and contraindications to minimize risks to patient health.
- Collaborate with healthcare professionals, including physicians, nurses, and pharmacists, to optimize patient care by tailoring medication therapies to individual needs.
- Maintain accurate and comprehensive patient medication records, documenting all prescribed treatments, dosages, and any changes to therapy for ongoing care and safety monitoring.
Requirements:
- Experience in a clinical pharmacy setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Excellent organizational and time-management skills.
