We are seeking a highly skilled and experienced Pharmacist to join our team, specializing in ICU, medical, and surgical care. This role involves providing expert pharmaceutical care to patients in intensive care units, medical wards, and surgical departments. The successful candidate will play a critical role in ensuring the safe and effective use of medications, optimizing patient outcomes, and collaborating with multidisciplinary teams.

Responsibilities

  • Providing a clinical pharmacy service to Adult ITU and supporting other clinical areas such as surgery when required.
  • Demonstrating and applying specialist clinical pharmacy knowledge to plan, manage, monitor, and review drug therapy for patients.
  • Providing a comprehensive pharmaceutical service to patients, staff, and practitioners, including prescription monitoring, provision of drug information, evidence-based practice, and patient counselling.
  • Promoting the active involvement of Pharmacy within the multidisciplinary team, participating in ward rounds, influencing prescribers, and recommending changes to patients' treatment where appropriate.
  • Monitoring and implementing drug clinical guideline policies and coordinating all aspects of pharmacy input to adult intensive care prescribing.

Qualifications and Experience

  • Prior hospital experience.
  • Some medical/surgical experience.
  • Degree in Pharmacy (MPharm or equivalent).
  • Registration with the General Pharmaceutical Council (GPhC).
  • Strong clinical knowledge and expertise in pharmacotherapy.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong organizational and time management skills.
  • Commitment to continuous professional development and staying up-to-date with the latest research and best practices.
  • Experience in conducting medication reviews and providing drug information.
  • Knowledge of clinical guidelines and protocols.
  • Experience in quality improvement initiatives and audits.
