Clinical Ward Pharmacist to join our medical wards team in a busy 72‐beded rehab hospital. You will play a key role in ensuring safe, effective, and timely pharmaceutical care for inpatients.
Key Responsibilities:
- Medicines reconciliation on admission
- Clinical screening and review of inpatient prescriptions
- Supporting and processing timely discharges
- Working closely with the MDT to support and optimise medicines use
Requirements:
- Qualified and registered pharmacist with strong ward‐based experience
- Skilled in medicines reconciliation, discharge planning and TTAs, and clinical screening
- Demonstrated experience in managing physical health within inpatient settings
- Must be available throughout October and December to cover annual leave/holiday periods
- Able to work 9–5 daily, or flexible shorter hours across 5 days per week
