We are looking for a dedicated Band 6 Clinical Pharmacist to join our client's team in London. This role involves providing expert pharmaceutical care to patients, ensuring safe and effective medication management. The successful candidate will collaborate with healthcare professionals to optimise patient outcomes through comprehensive medication reviews and personalised treatment plans.
Responsibilities:
- Conducting medication reviews
- Providing clinical advice on drug therapy
- Ensuring the safe dispensing of medications
- Educating patients and healthcare staff on medication use
- Monitoring patient progress
- Participating in multidisciplinary team meetings
- Contributing to the development of clinical guidelines and protocols
- Engaging in continuous professional development activities
Qualifications and Experience:
- Bachelor's degree in Pharmacy
- Registered with the General Pharmaceutical Council (GPHC)
- Minimum of 2 years of experience in a clinical setting
- Strong knowledge of pharmacotherapy and clinical pharmacy practices
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Experience in conducting medication reviews and audits
