We are seeking a dedicated and experienced Locum Band 5/6 UCR Intermediate Care Physiotherapist to join our team in Willerby. The successful candidate will be responsible for providing high-quality community-based care to adult patients in their own homes. Experience with System 1 is advantageous.
What your responsibilities will be for this Physiotherapy role?
- Delivering comprehensive assessments and interventions for adult patients within the community.
- Managing a diverse caseload that encompasses a variety of clinical needs such as falls prevention, frailty, long-term conditions, and end‐of‐life care.
- Utilising a wide range of community clinical skills, including equipment management, core treatments, and assessments.
- Effectively responding to and triaging patient needs.
- Working independently in patients' homes, ensuring high standards of care are maintained at all times.
Requirements:
- HCPC registration
- Right to work in the UK
- Proven community experience
- Access to a personal vehicle for work purposes
- Experience with System 1 (desirable)
- Strong skills in responding and triaging, with a broad range of clinical community skills
- Experience in managing a caseload including falls, frailty, long-term conditions, and end‐of‐life care
