We are looking for a skilled and motivated Clinical Ward-based Pharmacist to join our team in Wakefield. This role is ideal for a professional with a passion for patient care and a background in mental health. The successful candidate will be responsible for providing comprehensive pharmaceutical care to patients, ensuring the safe and effective use of medications, and supporting the healthcare team with their expertise.
Responsibilities:
- Performing clinical pharmacist ward duties, including medication reviews, patient consultations, and collaborating with healthcare professionals to develop and implement treatment plans.
- Ensuring compliance with regulatory standards.
- Educating patients on medication use.
- Participating in multidisciplinary team meetings.
- Contributing to clinical audits.
- Supporting the training and development of pharmacy staff.
- Utilizing EPMA systems effectively.
Qualifications and Experience:
- Registered with the General Pharmaceutical Council (GPHC).
- Up-to-date with mandatory training.
- EPMA experience.
- Experience working on-site due to limited time for training.
- Desirable but not essential: Clinical diploma, prescriber qualification.
