We are seeking a highly skilled and experienced Ward-based Clinical Pharmacist to join our team in Westcliff-on-Sea. The successful candidate will provide comprehensive clinical pharmacy services on the wards, ensuring the safe and effective use of medications. This role is essential in maintaining high standards of patient care and supporting the multidisciplinary team in delivering excellent service.
Duties and Responsibilities:
- Providing clinical pharmacy services on the wards, including medication reconciliation, clinical screening, and patient counselling.
- Working closely with the healthcare team to optimize medication therapy, manage discharge medications, and ensure seamless transitions of care.
- Participating in ward rounds, supporting clinical audits, and contributing to quality improvement initiatives within the pharmacy department.
