Our client is seeking an experienced Band 6 Occupational Therapist to join their dynamic team in Crewe. This role involves working within an acute inpatient setting supporting patients through their recovery journey and ensuring safe timely discharges.
Responsibilities
- Provide high-quality occupational therapy interventions within an acute inpatient environment.
- Support discharge planning and ensure effective pathways for patient transitions.
- Contribute to admission avoidance strategies working collaboratively with the multidisciplinary team.
- Deliver patient-centered care adapting to individual needs and goals.
- Participate in weekend and extended hours (8:00 AM – 6:00 PM) subject to discussion.
Requirements
- HCPC registration as an Occupational Therapist.
- Proven experience in acute inpatient care.
- Strong understanding of discharge pathways and admission avoidance.
- Excellent communication and teamwork skills.
