Band 6 Occupational Therapist Community Services

Location: Bristol

Contract: Temporary

Hours: 4-5 days per week totaling 37.5 hours

Our client is seeking an experienced Band 6 Occupational Therapist to join their team in Bristol. This role involves working across acute and community settings supporting patients with complex needs and ensuring safe timely discharges.

Key Responsibilities:

  • Support discharge planning, prevent admissions and manage frailty cases.
  • Provide interventions for patients with falls risk, neuro and orthopaedic needs.
  • Work collaboratively with the multidisciplinary team to deliver patient-centered care.
  • Promote independence and functional recovery through evidence-based practice.

Requirements:

  • HCPC registration as an Occupational Therapist.
  • Proven experience in acute and/or community settings.
  • Valid driving licence and access to a car (essential for community work).
  • Strong understanding of discharge pathways and admission avoidance.
  • Excellent communication and teamwork skills.
