We are seeking a dedicated and experienced Band 6 Locum Physiotherapist to join our Discharge to Assess team at The Withywood Centre, Sirona Care and Health CIC in Bath. This full-time role is essential in facilitating the safe and efficient discharge of patients from hospital to their homes or other care settings, ensuring they receive the necessary physiotherapy support during this transition.
Responsibilities
- Assessing patients’ needs upon discharge.
- Planning and implementing appropriate physiotherapy treatment programs.
- Monitoring patients' progress.
- Working closely with a multidisciplinary team to ensure seamless care and support for patients.
- Providing individualized therapy sessions and adjusting treatment plans as necessary.
- Educating patients and their families about their care and rehabilitation.

Qualifications and Experiences
- Degree in Physiotherapy.
