We are looking for a Band 4 Locum General Rehabilitation Support Worker to join our Community Stroke Team at Barts Health NHS Foundation Trust. In this role, you will provide essential support to patients recovering from strokes in their home environments, contributing to their rehabilitation and overall well-being.
Your responsibilities will include:
- Assisting in delivering rehabilitation programs for patients as directed by the supervising physiotherapist or occupational therapist.
- Supporting patients in their daily activities, helping them regain independence and improve their quality of life.
- Performing manual handling and mobility assistance to ensure patient safety and comfort.
- Monitoring patient progress and reporting any changes or concerns to the clinical team promptly.
- Maintaining accurate documentation of patient interactions and progress in accordance with organizational policies.
- Working effectively and independently in the community, providing care to patients in various home environments across Tower Hamlets.
Qualifications and Experiences:
- Previous experience working as a rehabilitation support worker is essential.
