Remuneration: £43,500 – £50,000 – plus £1000 working from home allowance

Our Liaison Practitioner’s play a key part in our multidisciplinary team, providing clinical support across services. The role involves responding to clinical queries, conducting initial clinical triage, and undertaking risk assessment and management of patients presenting with acute mental health needs. The Liaison Practitioner will play a vital role in ensuring the safe, effective, and timely management of referrals, supporting patient safety and care continuity across virtual and partner services.

This is a home-based role (applicants must reside in the UK), though occasional travel may be required for face-to-face meetings at various locations within the UK.

As Our Liaison Practitioner you Will

• Provide clinical support to the Consultant Psychiatrists, Prescribers and any other team member within P-UK who may raise a clinical query in relation to a client.
• Deliver a service of assisting and advising the wider P-UK team for patients presenting with acute mental health difficulties. This will include, but is not restricted to:
• Advising P-UK staff on the most appropriate course of action to support patients experiencing mental health difficulties.
• Contacting patients to clinically assess their mental state and risk.
• Provide clinical advice, based on latest empirical evidence and compliant with P-UK policies and procedures, directly to patients to manage risks and current mental state.
• Record findings from the contact accurately and contemporaneously in the electronic clinical record.
• Screen Adult ADHD referrals where there are risk and clinical suitability concerns and follow the P-UK referral triage process including clinical risk rating at the point of triage.
• Provide Risk Management of patients open to P-UK as per the P-UK BRAG Risk Management process.
• Refer a patient to a PUK Psychiatrist or to the Emergency Services as necessary.
• Work collaboratively with Psychiatrists and other Health Professionals and staff within P-UK.
• Assist P-UK to continue to develop working relationships with the relevant NHS agencies and other Care Providers.
• Implement excellent standards of patient care using evidence based clinical interventions, making autonomous clinical decisions about own professional practice.
• Escalate clinical risks/concerns as per P-UK Escalation Process in a timely manner.
• Place an emphasis on informed choice within a patient partnership approach; ensuring full information is given to the patient regarding treatment/support options following NICE guidelines.
• Provide support, information and advice to patients and, where appropriate, with client consent, to their families/carers.
• Develop and maintain effective working relationships within P-UK, NHS services and non-statutory services to ensure a seamless, coordinated service to patients and users of P-UK.

To Succeed As Our Liaison Practitioner, You Will

• An excellent communicator with the ability to engage effectively with service users, colleagues, and other professionals. You have a strong understanding of legislation, policies, and best practices relevant to ADHD and wider mental health service provision. You demonstrate the ability to think creatively and critically, always working with professional integrity and honesty, supporting your sound knowledge and experience in mental health treatment planning and management.
• Able to provide direct clinical judgments and treatment decisions using highly developed skills and knowledge regarding clinical management of adults with mental health difficulties.
• Contribute to the continuing development of Psychiatry-UK’s clinical processes and services, as appropriate.
• Ensure that the patient’s interests are fully considered within the context of their care, actively participating in ethical, shared decision making within the multi-disciplinary team approach.
• Exercise autonomous professional responsibility for clinical decision making within the parameters and expectations of the P-UK service and NMC/HCPC Code.
• Undertake risk assessments and review safeguarding needs alongside treatment strategies and record this accurately within the care record actioning safeguarding referrals as required.
• Escalate clinical concerns in a timely manner following P-UK policy and guidance.
• Determine the most effective prioritisation of own time to deliver a high-quality service demonstrating good organizational skills.
• Maintain up to date knowledge of relevant legislation, national and Psychiatry-UK policies and other issues relating to ADHD and Mental Health patient management.
• Access regular clinical supervision from an allocated Psychiatry-UK Team Member.
• Access continuing professional development opportunities relating to the Liaison Nurse role.
• Demonstrate an open, inclusive, and respectful approach which values individuals.
• Demonstrate skill in motivating/directing self and others.
• Demonstrate skill in supporting/developing others and provide clinical supervision to team members as required.
• Uphold, support and promote the values, aims and vision of P-UK, acting with integrity and honesty and demonstrating high standards of personal conduct at all times.

Essential Qualifications, Skills And Experience

• Professional Registration: First Level Registered Mental Health Nurse or Occupational Therapist with active registration with the NMC or HCPC.
• Clinical Experience: Minimum of 3 years post-qualification experience in a relevant mental health setting, or clear evidence of safe and independent clinical decision-making in a mental health context.
• Continued Professional Development: Demonstrated commitment to ongoing learning and professional development.
• Proven experience in the provision of mental health care and/or working with neurodevelopmental conditions.
• Advanced IT literacy, with the ability to work remotely using digital platforms and systems.
• Recent (within the last 2 years) experience of working within or alongside multi-disciplinary or multi-agency mental health teams.
