Role Description
This is a part‑time, on‑site role based in Southampton, Fareham and Gosport for a Family Support Worker. The Family Support Worker will provide one‑on‑one support for individuals impacted by addiction within their family or close relationships. Responsibilities include:
- Conducting 121 sessions
- Coordinating personalised care plans
- Facilitating support groups
- Providing resources and referrals for external services
- Actively collaborating with other professionals to ensure comprehensive service delivery
Qualifications
- Strong foundation in Social Work and Family Services to deliver effective support
- Experience in Case Management and coordinating care for individuals or families
- Outstanding Communication skills, both verbal and written, to engage with clients and partners effectively
- Ability to work collaboratively with teams and external agencies
- Empathy and sensitivity in supporting individuals experiencing emotional or challenging circumstances
- Previous or personal experience in addiction or substance abuse support services is an asset
